| Take US Route 219 South from Lewisburg, WV to the Fairgrounds. Just as you reach the Fairgrounds turn left onto Fair Street. Go one block to the stop sign. Then continue through the stop sign and the office is a red brick building on the right behind the Long John Silver's restaurant that is on the corner. The office is on the upper floor of the building. |

| Assignment will be accepted on all claims for these companies. Please note that participation with insurance carriers is reviewed and applied for periodically. To be sure that your plan is valid at the time of your appointment, please confirm participation with your insurance plan with the office staff when you present your insurance card at the time of your appointment. Regardless of participation, insurance claim forms will be filed with all insurance companies for all visits. Fees are expected to be paid at the time of your visit. Should any problem arise with billing, please bring this to the attention of our billing staff and they will make every effort to resolve the difficulty. |
