Directions to Office
Interstate 64 Exit 21. From the east exit then turn left, from the west exit then turn right. Then, from either direction, go approx. ¼ mile and turn left onto ARH Lane. Travel 1 block to the hospital. The office is on the east end of the hospital at the far end of the parking area. Turn to the left and follow the parking lot to the far side of the hospital. The office is in the Emmett Medical & Surgical Clinic building.

Assignment will be accepted on all claims for these companies. Please note that participation with insurance carriers is reviewed and applied for periodically. To be sure that your plan is valid at the time of your appointment, please confirm participation with your insurance plan with the office staff when you present your insurance card at the time of your appointment. Regardless of participation, insurance claim forms will be filed with all insurance companies for all visits. Fees are expected to be paid at the time of your visit. Should any problem arise with billing, please bring this to the attention of our billing staff and they will make every effort to resolve the difficulty.
