Directions to Office
Interstate 64 Exit 24. From the east exit then turn left, from the west exit then turn right. Then, from either direction, go approx. 2 miles on Main Street into downtown Clifton Forge. At the first stoplight turn left onto Commercial Avenue. Go 2 blocks through the 2nd stoplight. The office is a red brick building in the next block on the left with parking on the left before you get to the office and parking is available on the street.

Assignment will be accepted on all claims for these companies. Please note that participation with insurance carriers is reviewed and applied for periodically. To be sure that your plan is valid at the time of your appointment, please confirm participation with your insurance plan with the office staff when you present your insurance card at the time of your appointment. Regardless of participation, insurance claim forms will be filed with all insurance companies for all visits. Fees are expected to be paid at the time of your visit. Should any problem arise with billing, please bring this to the attention of our billing staff and they will make every effort to resolve the difficulty.
