| Take US Route 219 South from Lewisburg, WV past the Fairgrounds. Stay to the right as Route 219 curves to the left, you will be on Davis Stuart Road. Travel ½ mile to the stop sign, then turn right, still called Davis Stuart Road. Travel 1/4 mile, past Valley Medical Assoc., and then turn left at the entrance to Greyrock Professional Park onto Dawkins Drive. Travel 1 block to the fourth building on the left after entering Greyrock Professional Park. The office is in the rear of the building on the lower level. |

| Assignment will be accepted on all claims for these companies. Please note that participation with insurance carriers is reviewed and applied for periodically. To be sure that your plan is valid at the time of your appointment, please confirm participation with your insurance plan with the office staff when you present your insurance card at the time of your appointment. Regardless of participation, insurance claim forms will be filed with all insurance companies for all visits. Fees are expected to be paid at the time of your visit. Should any problem arise with billing, please bring this to the attention of our billing staff and they will make every effort to resolve the difficulty. |
